Heel Pain is generally the result of faulty biomechanics (gait abnormalities) that place too much stress on the heel bone and the soft tissues attached to it. It may also result from an acute injury, chronic repetitive stresses, wearing worn footwear, or being overweight. Once diagnosis has been established by x-ray findings and symptoms, treatment can begin. In severe cases, a cortisone injection is very effective in relieving the pain. Milder cases can be treated with physical therapy modalities and taping. Oral anti-inflammatory medications may be prescribed as well. For chronic heel pain due to abnormal walking, orthotics are very effective in eliminating pain and preventing recurrence once the pain has subsided.
Neuromas are inflamed nerves usually between the 3rd and 4th toes however, may occur in other locations in the foot. They usually form from repetitive trauma, high heel shoes, narrow toe boxes, and sometimes from injury. These can be treated with cortisone injections, physical therapy, and modifying shoe gear. When these are not successful, surgery is an excellent option.
Warts are actually formed by viruses which become inoculated into the upper skin layer. The wart lies in the epidermis and survives by utilizing the capillaries present. They can be treated by strong acids, liquid nitrogen freezing, and surgical curettage.
Fungal Nails and Athletes foot are both caused by a dermatophyte. There is much literature linking genetic predisposition to these unsightly conditions. Nails which are yellowish and crumbly and skin which is scaly and red usually are typical. They may occur separately or together. A fungal culture can actually identify the specific pathogen responsible. Treatment may include OTC remedies. There are many prescription topical creams and lotions. More recalcitrant cases may be treated with oral tablets. Some of the medicines by mouth require monitoring of the liver blood test done prior to treatment and after one month of therapy. Results can vary. Early detection is very important to the prognosis.
Bunions and Hammertoes are usually due to genetic predisposition and are not caused by shoes. They are aggravated by abnormal fitting shoes. They can be very painful. Conservative treatment may include injections, debridement’s of painful skin, padding and strapping. Those cases which continually cause pain can be corrected by surgical intervention.
Gout is an extremely painful condition usually resulting in Podagra which is a severe swelling of the big toe joint. Most patients who take a diuretic for hypertension or low dose aspirin therapy are especially susceptible to this painful malady. It is caused by too much uric acid which settles mainly in the joint of the big toe although may occur in other locations. Pain while walking and while in the bed with the cover on which came suddenly is a good indicator of gout diagnosis. It can be treated with cortisone injections and/or medications. Blood tests are usually ordered as well.
